Researchers at the University of Granada and the university hospital Virgen de las Nieves in Granada have developed a new radiotherapy technique that is much less toxic than that traditionally used and only targets cancerous tissue.

This new protocol provides a less invasive but equally efficient cancer postoperative treatment for cases of cancer of the oral cavity and pharynx.

The study -conducted between 2005 and 2008- included 80 patients diagnosed with epidermoid cancer of the oral cavity and pharynx, who had undergone lymph node removal. The affected nodes were located by the surgeon during the intervention and classified into different risk levels. Classification allowed physicians to target the areas at a higher risk of recurrence. This way, neck areas at a lower risk of containing residual cancer cells were not irradiated. Researchers achieved both to minimize the side effects of radiotherapy, and to reduce treatment discontinuation, thus achieving the therapy to be more effective.

A Highly Toxic Treatment

Over 70% of oral and pharynx cancer treated with surgery require supplementary treatment with radiotherapy occasionally associated to chemotherapy, because of the high risk for recurrence and spread through the lymph nodes. Radiotherapy and chemotherapy are highly toxic, mainly due to the ulceration of the mucous membranes lining the oral cavity; toxicity leads may patients to stop the treatment, which significantly reduces the chances of cure.

After a three-year follow up, using this new technique, scientists achieved to reduce the volume of irradiated tissue in 44% of patients. By this new technique, irradiation of an average volume of 118 cc of tissue was avoided. A total of 95% of patients completed radiotherapy and presented significantly lower toxicity than patients treated with the traditional technique. Recurrence rates did not increase.

This study was coordinated by University of Granada professors Rosario del Moral vila and Jos Mariano Ruiz de Almodvar Rivera. The results of this study will be published in the next issue of the journal Radiation Oncology.

Extension sought for interim finance director ? The Benicia Herald

If Bill Zenoni is to continue as Benicia?s interim finance director, the City Council will need to obtain an extension from the California Public Employees? Retirement System as well as amend his services agreement, Administrative Services Director Anne Cardwell said.

In a Feb. 14 report to City Manager Brad Kilger, Cardwell wrote that when Zenoni was hired as finance director after the retirement last fall of Rob Sousa, he was retired and receiving CalPERS benefits.

That state agency lets retired members work temporarily for a CalPERS-covered employer for a limited time, so long as it does not exceed 960 hours, she wrote. But if the employment continues, the worker must be reinstated into the retirement system.

Zenoni is projected to reach that limit March 2.

?CalPERS regulations also allow for an extension of the 960-hour limit by adoption of a resolution requesting an extension,? Cardwell wrote.

Benicia is seeking a permanent successor for Sousa, she wrote, but she advised that person isn?t expected to be hired before Zenoni?s 960 hours are reached.

She is recommending that the City Council approve a resolution at its Tuesday meeting authorizing the CalPERS request and amending Zenoni?s contract to extend his employ until Sept. 30.

In other matters on that meeting?s agenda, the Council will consider accepting the work done at the James Lemos dive pool, through which old plaster was removed and replaced, the pool entry?s lighting improved and Americans with Disabilities Act compliance measures were taken.

Mike Dotson, director of Parks and Community Services, is recommending approval of the jobs, which cost $113,553.

Originally, the project was expected to cost $100,000, but after bids were received more funds were needed to complete the work, Dotson wrote. At its Dec. 6 meeting, the Council approved the transfer of $13,553 from one facility maintenance account to another to underwrite the project.

However, the job was completed at $105,051, despite having to spend an additional $1,800 on ADA-required handrails, Dotson wrote. The balance of $8,502 will be returned to its original account, which usually is used for park and playground improvements.

The project, awarded to Burkett?s Pool Plastering, was finished last month.

The Council also will be asked to make minor modifications to the table of local limits for pollutants found in pretreated wastewater discharge, a table found in the city?s pretreatment program.

Those modifications are required by the Regional Water Quality Control Board, Charlie Knox, director of Public Works and Community Development, wrote in a Feb. 3 report.

?The local limits table governs nonresidential discharges to the wastewater treatment plant,? he wrote.

To obtain a national Pollutant Discharge Elimination System permit for its wastewater treatment plant, Benicia is required by the Environmental Protection Agency and the Clean Water Act to have a pretreatment process, Knox wrote.

Its pretreatment program regulates the wastewater discharge from businesses so that city plants and their staff, members of the public and the environment can be protected from potentially toxic pollutants, particularly manufacturing byproducts, Knox wrote.

The pretreatment local limits list refers to certain pollutants and the maximum concentration they can be if the discharge is admitted into the city?s wastewater system, he wrote.

The Regional Water Quality Control Board requires Benicia to set technically based limits for those pollutants, and businesses and industries connected to the city?s wastewater system must adhere to those limits, Knox wrote.

Benicia established limits in 1990, and a recent look indicated no changes were needed in the concentration levels. However, footnotes were added to define how limits are calculated and analyzed, Knox explained. ?There were no changes made to the numerical limits themselves.?

Knox also will ask the Council to accept the Lake Herman Waterline Cathodic Protection project, which provides corrosion protection to three buried pipelines between the city?s water treatment plant, Lake Herman and Valero Benicia Refinery.

The project was budgeted at $120,901 and was accomplished for $119,378.05, Knox wrote Feb. 3. The money came from the Water Major Capital Projects Fund and the Water Capital Connection Projects Fund. Knox is recommending approval.

The Council also will be asked to replace a vehicle that has been assigned to one of the city?s K-9 officers.

Benicia police Chief Andrew Bidou said a Ford Utility Police Interceptor can be purchased from Downtown Ford Sales, through the Contra Costa County vehicle contract awarded to that company, for up to $31,365.47 using remaining California Supplemental Law Enforcement Service Fund money. That money must be used only for front-line police services.

The Interceptor would replace a 2006 Crown Victoria that has been driven more than 100,000 miles and needs more expensive repairs at accelerating intervals, Bidou wrote in a Jan. 19 report.

The Council also will consider on second reading amending its ordinance regulating Council meetings. The change would establish a 6 p.m. start for regular but closed session meetings and add the fourth Tuesday of the month as a regular meeting date. The starting time of regular meetings, 7 p.m., would not be changed by the amendment.

The Council will meet at 6 p.m. Tuesday in closed session to discuss a real estate matter. The regular meeting will start at 7 p.m. in Council Chambers of City Hall, 250 East L St.

WASHINGTON (AP) ? President Barack Obama says the painful restructuring of America's manufacturing base means a lot of jobs are gone forever, but that does not mean Americans must "settle for a lesser future."

In his weekly radio and Internet address Saturday, Obama said American manufacturers are reinventing themselves with new technology and new efficiencies that have helped lead to what the administration claims are 3.7 million new jobs created over the past two years.

"Factories where people thought they'd retire have left town. Jobs that provided a decent living have been shipped overseas" and will not return, Obama said. "But that doesn't mean we have to settle for a lesser future."

He recorded the address while visiting a massive Boeing Co. manufacturing plant near Seattle on Friday. He toured a modern factory floor and the inside of one of the aircraft maker's new flagship Dreamliner passenger jets.

"Companies like Boeing are realizing that even when we can't make things cheaper than China, we can make things better," Obama said in the weekly address. "That's how we're going to compete globally."

Delivering the Republican address, Washington Rep. Cathy McMorris Rodgers accused Obama of overspending that will set the country up for a financial implosion like the one in Greece.

"Instead of leading the effort to bring down our debt and make tough choices, the president is proposing that we spend more and more," she said.

While at Boeing on Friday, Obama announced steps to offer financing to U.S. companies to match help their foreign competitors get, with the goal of helping American industries compete.

Obama called on Congress to extend the Export-Import Bank's authorization. White House officials said the bank will reach its lending limit at the end of March and Obama pointed to it as a key player in helping promote U.S. exports.

At the same time, the White House announced that Boeing will participate in an Export-Import Bank program that helps companies advance money to suppliers on export-related contracts. Administration officials said Boeing would be committing to more than $700 million in short-term credit this year. Officials said the arrangement would help Boeing compete for foreign clients against European jet maker Airbus.

Facing re-election, Obama has pointed to a decline in unemployment and trumpeted a recent boost in manufacturing jobs as an indicator of an economy on the mend. Republicans seeking the White House have accused Obama of failing to steer the economy out of a deep recession.

Huge prison fire kills over 350 inmates in Honduras (Reuters)

COMAYAGUA, Honduras (Reuters) ? More than 350 convicts died screaming and spluttering when a huge fire ripped through a prison in Honduras, reducing large swathes of the complex to a blackened husk.

Choked by smoke and scorched by flames, hundreds of inmates scrambled to escape with their lives after an inmate apparently set fire to the overcrowded prison in Comayagua, about 75 km (45 miles) north of the capital Tegucigalpa on Tuesday night.

But 359 people were unable to get out and perished in the blaze, the attorney general's office said. It was one of the worst prison fires in history, a grim new milestone for a country the United Nations says is already the most murderous on the planet.

"I thought I was going to die," said Marco Alvarez, a 27-year-old serving time for armed robbery. "I was just thinking about saving my life. We shouted for the prisoner warden, but nobody came. Nobody would open up."

Inmates heard screams as the fire engulfed the prison, trapping inmates in their cells. Many of the bodies recovered from the gutted complex were so charred, officials said they would need to use dental records and DNA to identify them.

Furious relatives came to vent their anger outside the prison, which stands behind a fenced off field ringed with scattered palm trees where prisoners can plant crops.

Some of the protesters suspected it was a deliberate attack and that police and guards knew about it ahead of time.

"This was no accident," said Carmen Sapeda, 47, who came to see if her convict brother Wilfredo was alive.

"This was planned from inside the prison," she said as dozens of friends and family members threw sticks at police in a pickup truck and screamed "murderers" at them.

All day, rescue workers and police labored to remove bodies from the scene while angry relatives of the prisoners remonstrated with authorities.

The governor of Comayagua province, Paola Castro, said a convict had apparently started the fire.

"One inmate got in touch with me just after 11 p.m. to say another inmate had set fire to the prison in block number 6, presumably by setting fire to a mattress," she told Reuters, explaining that she had met the prisoner during her social work at the prison.

It was unclear why the inmate would have started the fire.

Honduras is ravaged by violent street gangs, brutal drug traffickers and rampant poverty. The violence on the streets is mirrored by frequent riots and deadly clashes between rival gangs behind bars.

SHOCKING

But the devastation wrought in the Comayagua prison was shocking even by Honduran standards.

By the time Red Cross volunteer Jose Manuel Gomez arrived, all he could do was gather up remains of some victims.

"We're placing them into bags in parts because when we grab them, they disintegrate," he said.

The inferno was the third major prison fire in Honduras since 2003 with dilapidated jails packed at more than double their capacity across the Central American nation.

Many of those who came to protest the events in Comayagua were women. Police responded to rocks being thrown by firing shots into the air and shooting tear gas at protesters.

President Porfirio Lobo said he had suspended the director of the prison and the head of the national penal system to ensure a thorough investigation.

He promised to "take urgent measures to deal with this tragedy, which has plunged all Hondurans into mourning."

Police reported that one of the dead was a woman who had stayed overnight at the prison and the rest were inmates, but noted some of those presumed dead could have escaped.

Alvarez, along with many other survivors, got out of the jail after forcing his way through the tin roof of his cell.

"There were five of us cellmates who escaped, and five who died," he told Reuters after limping out of the prison with a severe burn on his leg. "I don't know about the other five."

VIOLENT GANGS, DRUGS

Honduras' violent street gangs, known as 'maras', gained power inside Hispanic neighborhoods in the United States in the 1980s and then spread down into Central America. Their members wear distinctive tattoos and are involved in drugs and weapons trafficking, armed robbery and protection rackets.

The country is a major narcotics trafficking transit point for South American cocaine moving north to consumers in the United States, and authorities say they are grappling with a growing presence of violent Mexican drug cartels.

Honduras had more than 80 homicides per 100,000 people in 2009, a rate 16 times that of the United States, according to a United Nations report last year. A slow and inefficient justice system has stretched jails to bursting point.

All told, the country's penitentiaries are meant to hold 6,000. But the prison population is more than 12,500. Comayagua housed more than 850 inmates, well above its limit of around 500. Officials are still investigating what caused the fire.

In 2003, a blaze broke out after a riot in another prison in northern Honduras, killing 68. An investigation found police and prison staff had shot and stabbed inmates in the melee.

The government pledged to improve the crumbling prison system but just a year later more than 100 prisoners were killed in a fire in San Pedro Sula. Survivors of that blaze said guards fired on inmates trying to escape or left them locked up to die.

A political crisis ripped through Honduras in mid-2009 when a widely-condemned coup toppled the democratically elected president but the country has been trying to heal divisions since Lobo was elected later that year.

If watching Newt Gingrich during the race for the Republican presidential nomination has you longing for political days gone by, you'll want to tune into PBS on Monday for "Clinton."

Former President Bill Clinton is the subject of the new two-part, four-hour documentary, but Monica Lewinsky -- the intern at the center of his 1998 impeachment scandal -- will likely generate the most buzz. Interviews with former advisors?shed new light on Clinton's thinking when it came to the 23-year-old who nearly ended his presidency.

The Guardian reports?that?loyal Clinton adviser Dick Morris revisits?the moment Clinton told him just before evidence of his affair with Lewinsky was about to be made public.

"Bill said to me: 'Ever since I got to the White House I have had to shut down my body,' " said Morris, adding that the president told him he had been weak regarding Lewinsky and had done enough with her to be in serious trouble.

Robert Reich, the former labor?secretary, is another of those who worked alongside Clinton and was shocked by the affair. "He would not be so stupid as to jeopardize his whole presidency, I felt," Reich said. "That was not the man I knew."

Lewinsky herself is not interviewed in the program.

The Los Angeles Times notes that Lewinsky, who left the U.S. for London in 2005, has kept a very low profile in recent years.

"Clinton" airs Monday at 9 p.m. on PBS.
